  Responsibilities of the PAS Caregiver include, but are not limited to, the following:

  1. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
  2. Provide companionship through discussion, reading, writing letters and helping communicate with friends and family online.
  3. Run errands on behalf of the client
  4. Assisting the client with short distance outings such as salon and doctor appointments, shopping and social events
  5. Light housekeeping such as vacuuming, dusting, washing dishes, changing bed linens and laundry
  6. Meal preparation and feeding
  7. Assist client with dressing, routine hair and nail care (non-diabetic)
  8. Assisting in ambulation and exercise as instructed by the primary care nurse or therapist.
  9. Provide normal range of motion and transfers/positioning as directed.
  10. Assistance with self-administered medications as allowed by state regulations, such as “reminders.”
  11. Providing respite for patient's/family/caregiver when on-site, as appropriate.
  12. Keeping patient's living area clean and orderly, as assigned.
  13. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.
  14. Respectful of patient and family/caregiver environment and patient's personal needs.
      
• Part-Time Pharmacy Delivery Driver (Decatur, Tx) ↓

  ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  1. Reviews and verifies accuracy of delivery manifests (sheet(s)/tickets) and other office documentation.  In collaboration with Pharmacy staff prioritizes items for delivery.
  2. Professionally drives vehicle for pick-up and delivery services. Delivers items to specific locations according to priority requests from Pharmacy personnel.
  3. When on duty with Solaris Pharmacy, incumbent must have cell phone on and be available for emergency calls and/or service.
  4. Provides Pharmacy Delivery Services by ensuring prompt and accurate distribution of medications and supplies to Solaris offices and medical care providers in accordance with customer demand and the requirements of the assigned route.
  5. Provides timely and accurate completion of deliveries to Solaris Pharmacy customers while using the most safe and efficient delivery routes.
  6. Maintains the order of assigned work area (if applicable).
  7. Maintains company vehicles and equipment (if applicable) in proper working condition and performs minor roadside repairs. May have company vehicle repaired when approved by Pharmacy Director or his/her appointee.
  8. Performs general pharmacy clean up, including vacuuming, breaking down boxes, disposal of trash and general custodial type work as scheduled or upon request of pharmacy personnel.
  9. Assists pharmacy personnel by restocking supplies, containers, bottles, etc. on request.
  10. Maintains accurate log books and vehicle inspection records.
  11. Assists in training newly hired Pharmacy Delivery Service Assistants as to safe driving procedures, routing, paperwork, etc. when requested.
  12. Obtains and submits all bills and receipts for payment or reimbursement to designated Pharmacy personnel on a timely basis.
  13. Performs clerical function incidental to delivery activities, including maintaining and logging orders for pickup and delivery; Sorting/filing of delivery/shipping manifests as requested by Pharmacy Director or his/her designee.
  14. Performs any other duties as required to ensure a safe and efficient pharmacy operation

  The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

  Patient Care

  1. Completes an initial and comprehensive assessment of patient and family to determine hospice needs.  Provides a complete physical assessment and history of current and previous illness.
  2. Provides professional nursing care by utilizing all elements of nursing process.
  3. Assesses and evaluates patient’s status by:

  • Writing and initiating plan of care
  • Regularly re-evaluating patient and family/caregiver needs
  • Revising the plan of care as necessary

  1. Initiates the plan of care and makes necessary revisions as patient status and needs change.
  2. Uses health assessment data to determine nursing diagnosis.
  3. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.
  4. Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  5. Counsels the patient and family in meeting nursing and related needs.
  6. Provides health care instructions to the patient as appropriate per assessment and plan.
  7. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
  8. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

  Communication

  1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.  Records pain/symptom management changes/outcomes as appropriate.
  2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  3. Communicates with community health related persons to coordinate the care plan.
  4. Teaches the patient and family/caregiver self-care techniques as appropriate.  Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.  Works in concert with the other hospice Interdisciplinary Group Members.
  5. Provides and maintains a safe environment for the patient.
  6. Assists the patient and family/caregiver and other team members in providing continuity of care.
  7. Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
  8. Attends Interdisciplinary Group meetings.

  Additional Duties

  1. Participates in on-call duties as defined by the on-call policy.
  2. Ensures that arrangements for equipment and other necessary items and services
     are available.
  3. Supervises ancillary personnel and delegates responsibilities when required.
  4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
  5. Fulfills the obligation of requested and/or accepted case assignments.
  6. Actively participates in quality assessment performance improvement teams and activities.
  7. Adheres to the Standards of Conduct
